Benefits Available by Employee Work Status
Legal Holidays - The Hospital pays full pay for 6 legal holidays per year (prorated for part time employees). There is no waiting period for Holiday Pay. HoliEligible employees are paid one and one-half times the hourly rate if worked plus additional day off. Refer to HR policy 935-10 for details.

Paid Time Off - Employees are eligible to use after 6 months. Full time accrual is based on budgeted hours, part time accrual is based on actual hours worked. Both full time and part time accrual hours are based on years of completed service per the below chart. Refer to HR policy 935-30.
Health Insurance - Eligible employees may choose from Blue Choice or Preferred Care Products. Hospital contributes toward a portion of the cost for full time employees. There is no contribution for part time employees under 75 hours per pay period. Effective 1st of month following eligibility date.
Benefit in lieu of Health Insurance . Full time employees, who do not participate in the Hospital's health insurance benefit, may sign up to receive a $60.00 per month benefit in lieu of the Health Insurance. Employees must show proof that they have other coverage outside of our plan in order to participate. This does not apply to part time.
Dental Insurance - Eligible the 1st of the month following 6 months of full time or part time employment. Full time cost is $5.00/family or $2.50/single per pay period. Part time employees may pick up a single policy for themselves at $5.00 per pay period w/reduced benefits.
Basic Term Life Insurance - Full time and part time employees (35-80 hrs) are eligible after completion of 3 months of employment. The Hospital will provide life insurance and accidental death and dismemberment ins. = to annual salary to next highest $1,000. Premium paid by the hospital.
Optional Term Life Insurance - Optional term life insurance can be added to the Term Life policy that is provided by the Hospital. Employee pays 100% of the Optional Term policy premium at a discounted rate.
Tuition Assistance . Employee's status must be budgeted at least 35 hours per pay period; eligible after 6 months of employment. (See Human Resources Policy 935-28)
Supplemental Whole Life Insurance (SLI) . Whole life insurance is available for employees through the Farmington Company, 1-800-621-0067. The premium can be paid through payroll deduction
Tax Sheltered Annuity Program - Voluntary retirement savings program. Contributions are deducted before taxes. Three company options are available: Lincoln Life, (guaranteed interest and variable funds), Diversified Investment Advisors (fixed, Gov't fixed and keynote series) and IDS American Express (fixed and variable). Available upon employment.
Retirement Savings Program - 401K plan - Eligibility requires that employee's complete 1 year of employment, must be over 21 years of age, and completed over 1000 hours of service. The Hospital will contribute up to 2% of employee's bi-weekly salary, along with the employee's contribution.
College Savings Plan . NYS 529 plan available. See Human Resources for details on enrollment.
Short-Term Disability Insurance - Protection for off-the-job injury or illness. A 7-day waiting period is applied. Supplemental disability pay benefit after one year of service. See Human Resources for details on qualification.
Long-Term Disability Insurance - The Hospital covers Exempt employees. Optional Long Term Disability policies are available to non-exempt employees through the Farmington Company. Call 800-621-0067 for details.
Worker's Compensation - Protection for on-the-job injuries provided by the hospital. An Employee Injury Report must be filed with the employee's department manager. Contact Human Resources immediately upon injury.
Travel Insurance - Upon employment each employee is covered by $100,000 travel/accident insurance while traveling on hospital business.
Subsidized Child Care - Our Children's Place, located behind the Clifton Springs Methodist Church, Main St., Clifton Springs, is available for children of full time and part time employees. Call 462-0460 for details on pricing and availability. A 15% discount will be applied.
Pharmacy Discount - OTC and prescription discount given to employee and immediate family.
Cafeteria Discount - All employees may purchase food at reduced prices.
Parking - Free parking at appropriate lots, require parking tag.
Direct Deposit - Available at any bank.
Finger Lakes Federal Credit Union - Federal savings & loan association w/direct deposit through payroll deduction.
Flexible Spending Account - Tax-free deductions on non-reimbursed medical and dependent care expenses if eligible.
Homeowners, Rental, and Auto Insurance - 5% discount and payroll deduction through Liberty Mutual Insurance.

Malpractice Insurance - Paid by the hospital, all employees covered while working.
